Overview:
CPST is a face-to-face intervention with the individual, family or other significant peers provided on a one on one basis.
This program is designed to help individuals with serious mental illness to achieve stability and functional improvement in the following areas: daily living, finances, housing, education, employment, personal recovery and/or resilience, family and interpersonal relationships and community integration.

Provide mobile treatment and rehabilitation services to individuals who have difficulty engaging in site- based programs who can benefit from off-site rehabilitation

Work with the individual, their family members, and other significant peers to identify strategies and treatment options specific to the individuals needs with the goal of minimizing the negative effects of mental illness symptoms, emotional disturbances, or associated environmental stressors which interfere with the individual’s treatment goals.
- Provide supportive counseling, solution-focused interventions, emotional and behavioral management solutions, and problem behavior analysis to the individual and others involved.

Assist the individual with independent living skills, both at home and in the community, to promote recovery and growth.

Provide ongoing rehabilitation support for individuals pursuing employment, housing, or education goals.

Work with the individual and others to address functional deficits associated with their mental illness by utilizing strengths-based planning and treatments.
- Assist the individual with effectively responding to or avoiding identified precursors or triggers that would risk their remaining in a natural community location

Applicant should possess one of the following:

License in Mental Health Counseling

License in Social Work (LMSW, LCSW, LCSW-R)

Registered Nurse
-
License in Marriage and Family Therapy

Licensed Psychologist
-
A nurse practitioner in psychiatry

Physician assistant
-
Psychiatrist
In addition to the professional license, an ideal applicant should have at least one year of experience in mental health and/or substance use counseling.
