Job Description
Hearth's Supportive Housing site in Boston's South End is looking for both per diem as well as a PT benefitted weekday PCHM (M,T,W 7:30am-2:30pm). Hearth's Supportive Housing site in Dorchester (off VFW Parkway - plenty of parking) is seeking per diem Home Health Aides/Personal Care Homemakers to cover shift(s) while an employee is on vacation, holiday or unplanned time off. HHA/PCHM shifts are weekdays from 7:30am-2:30pm or 2:30pm-10:30pm or may vary slightly 7-3 or 3-11. Each shift is 8 hours. There is an ongoing need for per diem HHA/PCHM at Hearth sites.
Hearth is dedicated to ending homelessness among the elderly. Hearth also has other supportive housing facilities in Jamaica Plain, Dorchester and Brookline, as well as an Assisted Living Facility at Ruggles in Roxbury.
Hearth requires all employees to be vaccinated for COVID-19 plus booster when eligible.
Major Functions:
- Gives or assists with personal care services as outlined in service plan.
- Provides household services.
- Observes records and reports pertinent information to team regarding tenants.
- Promotes and encourages independence.
- Records in daily personal care log.
Required Skills, Abilities and Knowledge:
- Must be able to read and understand English.
- Constantly able to communicate specific and general concepts verbally, by phone or in person, using varying levels of vocabulary.
- Constantly able to lead, organize, and delegate tasks on a program level.
- Ability to work with individuals and families from diverse ethnic, cultural and social backgrounds.
- Frequently able to remember multiple tasks/assignments given to self and others during varied periods of time.
Required experience:
- Prior CNA or PCHM experience.
- Minimum of 1 year working with elderly, nursing home settings, or group home
- CPR certified (must be current)
